The answer to your problem is a simple one.....
There is no such thing as "fixing the loose butterfly screws" issue once it happened, as this means that you only got half a fix done and in all cases where such a patch-up job was done, the xtrail would suffer from consequent problems.
In ALL cases on the loose butterfly screw, the only real fix is to have the engine replaced, as the damage caused to the cylinder head is far too great to be repaired back to the factory values. Rebuilding the head is NOT the answer at all and I bet if you do a compression test of that rebuilt engine you would see that it still has problems.
Had you come here to our forum seeking advise when you had the butterfly problem, we would have advised you against re-building the head and wasting money on such a repair job when you could have spent that money on buying a 2nd hand engine with no problem. That is the only logical answer to your issue if you still want to keep driving the car hassle free.

That's right Danielle and I believe the issues you are having now are directly related to the damage caused to Cylinder #1 and it has been damaged beyond repair because of the screws.
As I said, if you want to keep the car, start shopping around for a 2nd hand engine.

If you want to double check that your cylinder is damaged, find a mechanic with a bore scope (small camera on a flexible shaft) to insert it into the engine and have a look for signs of damage.
With a bit of luck it might be something else that is causing the engine to do what it is doing, but as Jalal said, it doesn't sound promising.
