Difficulty taking off

Hi all.
I have a little problem with my t30 2.5.
It's a 5 speed manual and it's been having the old flaring issue between gears since I bought it.
I have read a lot of posts in here and replaced the cam and crank sensors with new genuine ones the round steel type.
I have replaced the spark plugs and checked the pcv valve for functioning.
However the flare has reduced from 1000 rpm to now about 200 to 500 rpm.
It feels like there is a delay for a second or two after I release the pedal.
It will still continue to hang on idle at 1500 rpm for a couple seconds when free revving then return to idle at 900rpm and be it a little rough idle.
The main problem is now when I take off I have to rev the engine to 1500 or 2000 then release the clutch or it will stall.
It's a pain to drive I've driven many manuals and none have been like this and was wondering if anyone knows of a possible solution?
I'm kinda thinking toe or throttlebody? Do these items when faulty produce these problems?

Also note I have undertaken idle relearn successfully.

As you have a manual version, you appear to be describing the symptoms of a worn out clutch.

2011/12 Australia:- 2.5 litre petrol 4WD, Series IV ST, CVT : + some gadgets and cosmetic add-on's. Dirt, (it protects the paintwork) X-2429(c).
