I think you should. Refer to THIS flush procedure if in doubt and make sure you use the correct coolant type as recommended by Nissan.
This guide also suggests replacing the radiator cap if sludge is noticed in the spring area of the cap, which is what you have, so maybe try replacing the radiator cap first before you do another flush.

When you clean drained it, did you remove the block plug ?

Hi Keith,
Here is the xtrail specific instruction for changing the coolant and flushing teh system:
In regards to using coolants other than Nissan, I wouldn't risk it, especially when the price difference between a Nissan one and another brand is not that huge and especially if you're still under the factory warranty, so that you don't give Nissan any excuse if something goes wrong.

Opposite end to the gearbox and on the firewall side of the engine. Probably go up from underneath....
